This discussion will focus on Post-Mastectomy Pain Syndrome (PMPS)—its incidence, underlying mechanisms, and modern reconstructive strategies aimed at nerve preservation and neuroma prevention. The session will highlight the evolving role of Regenerative Peripheral Nerve Interface (RPNI) and Targeted Muscle Reinnervation (TMR) techniques in managing chronic neuropathic pain following mastectomy and other reconstructive procedures. A brief review of RPNI, which involves implanting transected nerves into free muscle grafts to provide physiologic targets and prevent neuroma formation, and TMR, which reroutes available sensory nerves into nearby motor branches to establish reinnervation to minimize pain, will be included. These emerging microsurgical approaches demonstrate promising results in improving pain outcomes, reducing opioid dependence, and enhancing quality of life for reconstruction patients.
